Custom form builder for Canvas
For instructors: Click + New Form to create a form. Choose a template or start blank. Drag field types from the left panel. Click any field to edit its properties.
For students: Click Fill Out → on any published form. Your progress is auto-saved as a draft. Submit when complete.
Templates included: Clinical Hours Timesheet, Patient Care Log, Observation Checklist, Peer Evaluation, Reflection Journal.
Field types: Text, Textarea, Number, Email, Date, Time, Radio, Checkbox, Dropdown, Signature, File Upload, Section headers, and Conditional Logic.
